Gate Tag Registration Form (Approved 4/1/2026)  Print and fill out by hand ​

ONE FORM PER VEHICLE. Please complete this form and turn it in at the Member Services desk in the Madera Clubhouse.
 
  1. HOMEOWNERS: The first two tags issued are free when you become a Quail Creek Resident. Additional tags are $30.00 regardless of the reason a tag is needed.
  2. RENTERS: $30.00 per vehicle. Please submit the completed form to Member Services only.
  3. If you are a resident, your Quail Creek member account will be charged. If you do not have charging privileges, you will need to pay with a check, credit card, or cash at Member Services.

A representative will contact you when the new tag is ready for pick up. This process typically takes 7-10 business days following form submission.
